I am writing to inform you that I will be retiring on December 31, 2026.

​

As I have already notified the Régie de l'assurance maladie du Québec (RAMQ) of my retirement, we encourage you, if you wish, to register now with the Québec Family Doctor Finder (GAMF) to facilitate your future care with a new physician. You may also access the Primary Care Access Point (GAP) by calling 811, option 3.

​

As long as you remain registered with our clinic, you will continue to have access to our services. If you wish to deregister before December 31, 2026, you must contact the RAMQ and complete the process online. You will automatically be deregistered on December 31, 2026.

Over the past several months, we have made every effort to recruit new physicians to ensure continuity of care for our many patients, but unfortunately, these efforts have been unsuccessful. It is therefore with great regret that I must inform you that I will not be able to transfer your care to another physician at our clinic, as all physicians and nurse practitioners will cease practising there as of December 31, 2026.

We have also attempted to reach agreements with other medical clinics to arrange for the transfer of our patients. Unfortunately, to date, no definitive agreement has been reached to ensure continued care for our large patient population.

Regarding the transfer of your medical records, please send us a signed request for the transfer of your records, along with the contact information of your new medical clinic. If you have not found a new physician by December 31, 2026, your medical records will be transferred to the Collège des médecins du Québec. Your new physician or medical clinic will then need to contact the Collège directly to obtain them.

In the months following December 31, 2026, I will continue to receive the results of tests and consultations you undergo and, when necessary, will contact you to discuss them.

​

Your test results are also available through the Québec Health Booklet (Carnet santé Québec). If your medical condition requires attention while you are waiting to find a new physician, you may call 811, option 1, to obtain a medical appointment.

​

For any prescription renewals, forms, or documents requiring completion, we ask that you submit them to the clinic's reception desk or send them electronically. We will contact you if additional information is needed.

For any medical emergency, please call 911. For non-urgent medical concerns, please call 811. You may also access the Primary Care Access Point (GAP) by calling 811, option 3.
